Policy Statement

Support Home Health care LLC will accept patients for home health services upon a reasonable expectation that the agency can adequately and safely meet each patient's medical, nursing, therapeutic ad social needs in their place of residence. This policy is applied consistently to each prospective patient referred to for home health care.

Policy Compliance Check

Ensuring adherence to established acceptance to service criteria

Purpose

To establish clear criteria and guidelines for accepting patients into service, ensuring compliance with federal regulations, specifically 42 CFR 484.105(i)

Acceptance criteria

  1. Physician or healthcare provider's certification:

All admissions require a physician's or healthcare provider's order and certificatio of medical necessity for home health care

  1. Assessment of Needs:

A registered nurse or qualified therapist will complete a comprehensive assessment (OASIS-E1 if applicable) to determine the patient's medical, functional, psychological and environmental needs.

  1. Agency capacity

Acceptance will occur only if the agency has adequate staffing, resources and capability to meet the patient's identified needs effectively and safely.

Patients must reside within the agency's defined geographical service areas of 5 illinois counties (Cook, Dupage, Kane, Lake and McHenry).

Insurance coverage or source of payment will be verified prior to admission.

Support Home Health Care LLC accepts and provides services without regard to race, color, age, sex, religion, national origin, disability or sexual orientation.

Plan of care: All patient care will be provided based upon written physician (or healthcare provider) approved plan of care, reviewed and updated regularly in collaboration with the physician/ healthcare provider and interdisciplinary team.
